Sports Physical Information

Chippewa Valley Orthopedics and Sports Medicine will be providing sports physicals for Memorial High School students and incoming freshmen on May 13th from 5:30-6:30 at Chippewa Valley Orthopedics and Sports Medicine's Eau Claire Office.  The address is 4212 Southtowne Dr. Eau Claire.  Please see the attached flyer under "forms" on the right hand side for more information.

Code Signing Information

Fall sports code signing is set for Wednesday, May 22nd.  You can find the informational flyer under "Forms" on the righ side of this page.  Athletic fees can be collected the night of code signing at Memorial High School.

Code Signing Materials:

All Students participating in athletics at Memorial need to view the code presentation (#1) and print, sign and return the code receipt (#3 highlighted item).  However, you only need to do this once per academic year.  Therefore, if you signed the code for a fall sport you do not need to do it again.

**You can turn in the athletic code receipt the evening of code signing, or drop it off in the athletic office after code signing. 

Concussion links: The highlighted link is the form to be turned in by the parent and athlete.  This form needs to be on file for every sports season.  If you play a sport in the fall, winter, and spring you'll have 3 concussion forms on file in the athletic office by June.

ECASD High School Athletic Fees

For the 2012-2013 school year, all high school athletes need to turn in a completed fee form and all fees need to be paid prior to the start of Fall sports. Forms are available in the athletic office, online, and at the Administration Building. 

All athletic forms and fees must be turned in to the Administration building after July 1, 2012.  Please submit the fee form and a payment of cash or check (made out to ECASD) to the Accounting Department at the Administration Building, 500 Main Street (please do not send cash through mail).  Administration Building summer office hours are Monday through Thursday, 7:00 am – 5:00 pm. Cut-off dates for sport refunds include the following dates: Fall - September 14,   Winter - December 7,  Spring - April 12.  You will be reimbursed via mail within two weeks of these dates.
