All Students who will be involved in Athletics at Memorial must:

1.  View the Athletic Code Presentation

2.  Read and Understand the Athletic Code

3.  Sign and Return this form

** In order to process your form in a timely fashion, it is recommended that you return your signed form by August 1, 2012.   Return to the Athletics desk in Student Services at Memorial High School.
